Antonio Conte has won the Premier League Manager of the Month award for December.

The Italian boss led Chelsea to six straight wins over the month to extend their lead at the top of the table.

Conte also won the award in October and November and becomes the first manager to win the prize three times in a row.

Chelsea opened the month with an excellent victory at Manchester City as Willian, Diego Costa and Eden Hazard found the net.

Talisman Costa was again on target to see off West Brom before Cesc Fabregas scored the winner at Sunderland.

Chelsea made it three 1-0 wins in a row at Crystal Palace with Bournemouth then comfortably seen off on Boxing Day.

Stoke ran the Blues close on New Year's Eve but Willian scored twice in a 4-2 win.

Despite their stunning end to the year, 2017 got off to a disappointing start for the Blues as their unbeaten run came to an end against Tottenham.
